A Structured Medication Review is a detailed, personalised review of your medicines with a Clinical Pharmacist. It gives you an opportunity to discuss how your medicines are working, whether you are experiencing any side effects or difficulties, and any questions or concerns you may have about your treatment.
During the review, the Clinical Pharmacist will look at your current medicines and consider whether they are still appropriate, effective and safe for you. They may also discuss monitoring requirements, how and when you take your medicines and whether any changes could make your treatment easier or more effective.
Structured Medication Reviews are particularly useful for patients taking several medicines or those with more complex medication needs. Patients who would benefit from a review may be contacted by the practice and invited to arrange an appointment.
