We are pleased you have booked a holiday - however, travelling abroad can bring many health issues which can be problematic. To provide you with advice we need you to complete the form below and return it to reception ASAP - at least 6 weeks before your departure date.

We are pleased to announce we will be offering travel vaccinations again 

It is important to make the initial appointment as early as possible  - at least 6 weeks before you travel, 12 weeks dependent on your itinerary - as a second appointment may be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

If your itinerary is complex i.e. not a package holiday and especially if it includes Africa or South America we need the form returned at least 12 weeks before departure.
